Patient Information
As prosthodontic specialists, we frequently work with other dental practices. If your dentist has referred you to us, rest assured our staff will work closely with his or her office.
Dentists and Doctors: If you are a dentist or doctor and have a patient in need of prosthodontic care, please call our office to arrange for a consult. If working together we determine the need for further diagnosis and treatment, our staff will work with you to ensure your patient gets seen quickly and receives the highest level of care, and that you are kept informed and involved in the process.
Financial Arrangements
We ask that payment be made at the time the service is rendered for visits such as initial exams, recall exams, x-rays, cleanings, appliance adjustments, fillings, crown re-cementations, and denture repair. For more complex treatment, we offer the following payment options:
- Full payment at first visit earns a 5% courtesy discount adjustment on half of the total.
- If not paid in full, half of the total is due on the first visit and the second half is due on the insert appointment.
- Outside financing is also available with CareCredit. CareCredit offers 3 and 6 months free financing or 24-36-48 & 60 extended financing. Our office will be more than happy to help you get approved for this program. http://www.carecredit.com
- No other discounts apply if you choose the second or third payment option.
- For your convenience our office accepts: Visa, MasterCard, Discover, and American Express.
We will submit insurance claims for you, as a courtesy to you. Patients with dental insurance are responsible for all dental charges since the insurance agreement is between the patient and his/her insurance company. If an insurance payment is received by our office, and your balance has been paid in full, our office will reimburse the insured. Any discrepancy (deductible, UC&R, etc.) between our fees and the amount the insurance carrier will pay, is the responsibility of the patient or guarantor of the account.
